Please enter your contact information, select the type of appointment you are requesting (Project Review or Eagle Board)
and then scroll to the 4th Thursday date of the month you would like for your appointment to take place then select from
the available appointments (shown by the green checkmarks).

You can also select your preferred date by clicking on the calendar icon and selecting the 4th Thursday of the month.

If there are no checkmarks available for the date you selected, then there are no available appointments remaining for
that particular month. Select the next month to search for available appointments.
